Note: This form can ONLY be used to confirm your dog registration details, as specified on the Dog Registration Form you received via email or post.

If all details are correct, you can sign the declaration and proceed directly to payment. Once your payment has been received, you new dog tag(s) will be mailed to you.

DO NOT fill out this form if any details have changed during the registration year, for example:

  • Your dog has been desexed
  • Your dog has died, been sold or disposed of during the year

Please note these changes on the Dog Registration Form you have received, sign and return the form.

I confirm that the information displayed on the front and on the reverse page of the Dog Registration Invoice relating to the dog(s) in my care is correct and I do not own or have in my possession any other dog(s) over the age of 3 months that are not registered.
